The Community Wellness MyWellness Patient Portal is the heart of our Remote Patient Monitoring Plus (RPM+) program. It takes and stores the readings from your connected monitoring devices to provide your primary care provider (PCP) with data to better inform your care plan. In addition, you can communicate with your clinical coach and attend your monthly sessions within the app. Finally, it is the gateway to Stronger Together – our community platform that allows you to connect with clinical coaches and other patients, as well as earn points for participating in discussions and reading educational articles. Points can be redeemed for prizes that support your health & wellness journey.
